Hi — thanks for explaining your readings so clearly. What you’re describing is a very common and completely harmless condition known as White Coat Hypertension or BP Anxiety Response — let’s break this down for you What’s happening When you measure BP at home in a calm setting (130/80 mmHg), it’s normal. But in the clinic, it spikes (165/90 mmHg) — that’s because of anticipatory anxiety and nervous system activation during BP measurement. When the cuff tightens, your body’s “fight or flight” system (sympathetic response) kicks in — heart rate and BP go up. Once the cuff releases, the tension drops and BP normalizes. This is not true hypertension progression, and your doctor is right — these spikes are not dangerous as long as your home readings are normal. About your medication You’re on Telma 40 mg (Telmisartan) — that’s an excellent medicine for your age. It protects the heart, kidneys, and blood vessels and controls BP smoothly throughout the day. Since your home readings are fine, there’s no need to increase the dose or add another drug right now. What you can do to reduce anxiety spikes Here are some very effective tricks: 1. Practice “calm breathing” before BP check: Sit quietly for 5 minutes. Take slow deep breaths — inhale for 4 seconds, exhale for 6 seconds. This lowers adrenaline and normalizes BP readings. 2. Avoid caffeine or smoking 1 hour before check. 3. Take readings at the same time daily (morning and evening). 4. If home BP remains <135/85, you’re perfectly controlled — ignore clinic spikes. 5. Track BP average: 5–7 readings taken at home over a week are much more accurate than a single clinic reading. 6. Mild exercise, meditation, and salt reduction help stabilize the nervous system and reduce such fluctuations. When to recheck or adjust medicine If your home BP regularly exceeds 140/90 mmHg, Or if you develop headaches, dizziness, or palpitations — then it’s time to review medication. Otherwise, you’re already doing great on Telma 40 mg. Reassurance You’re not alone — nearly 1 in 3 people with mild BP elevation experience this anxiety-related surge. It’s psychological, not pathological. Your heart, kidneys, and arteries are not under any damage risk from these temporary spikes. The goal is to keep your average BP normal, not to chase every occasional reading. I can help you with a personalized BP tracking and anxiety-control plan — including breathing patterns, correct BP measurement posture, and how to interpret readings week by week. Its white coat hypertension, where you have a higher BP when recorded at a clinic or hospital setting, which is due to anxiety as you mentioned, but is within normal limits at comfortable settings. As you said the readings are near 130/80 mmhg, and hoping you are taking your hypertension medications regularly, it's normal. As your doctor also mentioned these spikes are normal, there is nothing to worry about. For your satisfaction, you can start recording your BP atleast two times a day, at home, in a comfortable setting, after a proper rest, sitting relaxed for atleast 15 to 20 mins, not to be done immediately after doing some exhausting activities, or after a lot of walking. Record the BP with a correct method, to avoid unreliable readings. If your BP reading are any time more than 160 above or even more than 100 below, recheck BP after 10 to 15 mins again after proper rest, till it returns within normal limits. Anytime the reading in more than 180 above or even more than 120 below, Make sure you recheck again after 1o to 15 mins, still if similar, visit a nearby clinic or hospital immediately. Irrespective of the BP readings, if any time there are complaints of severe headcahe, sudden neck pain and stiffness, blurring of vision, fainting episode or dizziness, difficulting balancing, difficulty using or sudden weakness/ tingling sensation in arms, legs, face, nausea vomiting, visit a nearby clinic/ hospital nearby. Note down the recordings for the day for 1 week, if any abnormal values noted continously, despite taking medications normally, visit your doctor with them.
