Sleep Disturbance and sleep aralysis

I am experiencing severe sleep disturbance with a highly irregular sleep pattern. I currently sleep from 7 AM–11 AM and 2 PM–5 PM, yet have persistent daytime sleepiness, low energy, and impaired daily functioning. I have had ~3 recent episodes of sleep paralysis involving inability to move, attempted awakening without success, and associated breathing difficulty. Past history of anxiety and depression; was on psychiatric medication for 2–3 years with initial improvement but discontinued 2 years ago due to side effects. I have attempted lifestyle changes (e.g., walking) and melatonin; however, melatonin results in sleep onset followed by awakening after ~2 hours. Kindly advise on possible causes and which doctor should I consult (neurology/sleep medicine vs psychiatry or MD).
