"Hello. I understand how distressing these symptoms can be. Based on your description of severe burning pain that lasts for hours after a bowel movement, it is very likely that you are dealing with an **Anal Fissure**, possibly accompanied by a **sentinel pile** (the 'skin tag' you mentioned). While external hemorrhoids can cause discomfort, the prolonged 'burning' after pooping is a hallmark sign of a fissure. **Immediate Management & Advice:** * **Sitz Bath:** Sit in a tub of warm water for 10–15 minutes, 2–3 times a day. This helps relax the anal sphincter muscle and reduces pain. * **High Fiber Diet:** Increase intake of green leafy vegetables and fruits to ensure stools are soft. Hard stools will worsen the tear. * **Hydration:** Drink at least 3 liters of water daily. * **Avoid Straining:** Do not spend long periods on the toilet or strain during movements. **Suggested Treatment Direction:** You may require a topical ointment containing a calcium channel blocker (like Diltiazem) or Lidocaine to help the area heal and manage pain. However, since you mentioned bleeding and severe pain, it is crucial to rule out other conditions. I strongly recommend you consult a **General Surgeon or a Proctologist** for a physical examination. They can provide a definitive diagnosis and prescribe the appropriate medicated creams or stool softeners. If you prefer Ayurvedic treatment, a consultation with a **Proctologist (Kshara Sutra specialist)** would be appropriate to discuss 'Arshas' or 'Parikartika' (fissure) management." ### **Important Notes for the User** * **Bleeding:** Any instance of rectal bleeding should be evaluated by a doctor to confirm the source. * **Medicine:** Avoid self-medicating with over-the-counter creams before a diagnosis, as some treatments for hemorrhoids can irritate a fissure. * **Chronic vs. Acute:** If this is not treated early, it can become chronic, making it harder to heal with just medicine.

Your symptoms could be due to anal fissure , rather than external hemorrhoids.pain could last longer depending on the depth of tear. You can try sitz bath, apply anobliss cream twice daily for a few days, consult a surgeon to determine the exact cause

This sounds more like an anal fissure with a small skin tag, which explains the severe burning and pain after passing stools. It’s quite common and treatable with the right care. You don’t have to worry, but since it’s causing significant discomfort, proper treatment will help you heal faster and prevent recurrence. I can guide you step by step to settle this feel free to consult.

Severe burning pain that lasts for hours after passing stool, accompanied by bright red blood, is highly characteristic of an Anal Fissure (a small tear in the lining of the anus). While you have identified a 'skin tag,' this is often a 'Sentinel Pile,' which is a secondary skin growth that forms at the edge of a chronic fissure. This pain-bleeding-tag triad suggests the condition has progressed beyond the initial stage and requires active medical management to prevent the need for surgery."
Next Steps
Sitz Bath: This is the most effective immediate relief. Sit in a tub of warm water for 10–15 minutes, 3 times a day. This relaxes the anal sphincter muscle and reduces the burning pain. Fiber & Stool Softeners: , you must keep your stools very soft. Increase water intake and use a natural fiber supplement (like Isabgol) to prevent further tearing. Topical Ointments: You likely need a prescribed ointment containing a local anesthetic (to numb the pain) and a calcium channel blocker or nitrate (to improve blood flow and heal the tear). Dietary Discipline: Avoid spicy foods and caffeine, which can irritate the digestive tract and make bowel movements more painful. Mandatory Surgical Consultation: Chronic fissures with sentinel tags often require specialized care. A formal consultation with a General Surgeon or Proctologist is mandatory. A physical examination (digital rectal exam) is necessary to confirm the diagnosis and rule out internal hemorrhoids or fistulas.
Health Tips
the 'hours of pain' suggest your internal muscle is in spasm, which prevents the tear from healing. Book a detailed consultation so we can prescribe the specific healing creams and muscle relaxants needed to close the fissure before it requires surgical intervention."

Based on your symptoms — severe burning pain after passing stool, bleeding, and pain lasting for hours — this is more suggestive of an anal fissure rather than just a simple skin tag or hemorrhoid. A skin tag around the anus is often a sentinel tag, which commonly develops with a chronic fissure. Important: * Do not rely only on self-diagnosis or OTC/Ayurvedic meds without evaluation * If symptoms persist >1–2 weeks, you should consult a surgeon for proper examination * Chronic fissures may require advanced treatment (laser or minor surgery) Most cases improve with proper treatment — but correct diagnosis is key.
Next Steps
Avoid constipation: Take a high-fiber diet (fruits, vegetables, plenty of water) * Stool softeners (if needed) * Warm sitz baths (10–15 minutes, 2–3 times daily) * Topical medications (like anesthetic + muscle relaxant creams prescribed by a doctor)
Health Tips
If bleeding increases, severe pain continues, or swelling increases → get evaluated early
