Hi. To decide if Physiotherapy can help or you need surgery you need to get mri done. Based on the findings one can decide if you need surgery or just Physiotherapy.

The way you describe it is a recurrent dislocation, the younger you are the more is the chance of it continuing to be the same. You will require a orthopaedic surgeon opinion. You will benefit from an MR arthrogram, which will define the pathology.
Next Steps
Visit orthopaedic surgeon
Health Tips
Recurrent dislocation will cause repeated damage to the cartilage, avoid them

You didn't specify any history of shoulder dislocation or recurrent dislocation As per your history try doing physiotherapy with strengthening exercises for stabilisation If your symptoms persist you will require to get a MRI done which will identify the cause and depending upon which an Orthopaedic surgeon will guide you further
