This presentation could be related to several different neurological pathways. To give you an accurate diagnosis, we would need to review your previous reports and conduct a thorough workup. I recommend scheduling an appointment.

Hello. The acute symptoms over the last 2 days (headache, low-grade fever, vomiting, and abdominal discomfort) suggest an acute viral or systemic infection / acute gastroenteritis. Additionally, the 6-to-7-month history of severe painful periods accompanied by vomiting points toward Primary or Secondary Dysmenorrhea. Clinical Assessment: Recent Symptoms (Past 2 Days): Acute fever with severe headache and gastrointestinal disturbances (vomiting, abdominal pain) requires symptomatic relief and ruling out acute systemic viral conditions. Chronic Period Pain (Dysmenorrhea): Severe cramps accompanied by vomiting every month for over 6 months should be evaluated to rule out underlying structural or hormonal factors such as endometriosis, adenomyosis, or pelvic inflammatory disease (PID). Symptomatic Management: For Fever & Headache: Tab. Paracetamol 650 mg after food (up to 3 times a day as needed) to relieve headache and fever. For Vomiting & Nausea: Tab. Ondansetron 4 mg taken 30 minutes before meals. For Severe Menstrual Pain: Anti-prostaglandin medications such as Tab. Dicyclomine + Mefenamic Acid during period days under medical prescription to relieve uterine cramping.
Next Steps
Ultrasound Scan of Pelvis & Abdomen: Essential to evaluate the uterus, ovaries, and pelvic structure for cysts, fibroids, or signs of endometriosis. Blood Work: Complete Blood Count (CBC) and Erythrocyte Sedimentation Rate (ESR) to check for infection or inflammation levels.
Health Tips
Hydration with Electrolytes: Since she is experiencing vomiting, ensure continuous oral hydration using Oral Rehydration Salts (ORS), tender coconut water, or clear soups to maintain electrolyte balance. Rest & Light Diet: Eat small, bland, easily digestible meals (toast, rice gruel, bananas) and strictly avoid spicy, oily, or heavy foods. Warm Compress for Period Pain: Using a heating pad or hot water bottle on the lower abdomen during period cramps provides significant local relief.

Your recurrent headache with fever and vomiting should not be self-treated without evaluation, especially if this is a new/severe headache. Painful periods with vomiting for the last 6–7 months may occur with severe dysmenorrhea or conditions such as endometriosis and should be assessed by a gynaecologist. If you develop severe/worst-ever headache, persistent vomiting, neck stiffness, fainting, weakness, confusion or visual problems, seek urgent medical care. You may consult me on Practo for a detailed assessment and appropriate next steps.

Need assessment rather than assuming they are period related. A consultation would help assess the cause and decide whether examination or further tests are needed.

The symptoms may be related to painful periods, as nausea, vomiting and headache can sometimes occur along with menstrual pain. However, since the pain has become severe and is recurring for the last 6–7 months, it is better to find out the exact cause rather than taking painkillers repeatedly.
Next Steps
Please consult a gynaecologist for a proper examination. An ultrasound of the pelvis may be advised depending on the examination and symptoms.
Health Tips
keep yourself well hydrated, take adequate rest and you can use a warm heating pad over the lower abdomen for relief. Avoid taking painkillers repeatedly without medical advice.
