Query in rabies antibody titre

2025-12-01 19:35:57
Hello doctor, Please find my attached report. I have one doubt i checked my rabies antibody test which came 3.7 EU and test is done on 25th nov 2025. Test is done after 0,3,7 doses. The 3rd dose is taken in 10th nov. Today taken the last dose. Test is done after 15 days after 3rd dose. My vaccination history below; Complete ID course July 2025 Two times Booster ID on September and October 2025 New exposure in nov took ID with ERIG started from 3rd nov 2025 end with 1st dec 2025. My question is, if this titer value drop below .5 after few days lets say 5 days. How much risk from last exposure? I mean the rabbies virus will reactive if titre drop after few days ? Also does this titre value come from Erig or from arv only? Am i safe sir ? Need booster ? The value showing in EU .. is it right ? The test is sone early or in right time ?

Hello, I think the titre will fall over the time, but once you complete your schedule you’re fully vaccinated. Nothing to worry about.

The antibody levels are already adequate. So simply finish the course of the vaccines as you have. No further tests are indicated. All the best. J G S R (online) clinic

Your antibody report shows a value of 3.7 EU/ml, which is well above the protective threshold of 0.5 EU/ml. This means your body has already developed strong immunity from the vaccination course, and you are fully protected from the exposure in November. It is completely normal for antibody levels to rise and fall over time. Even if the number drops below 0.5 in a few days or weeks, it does not increase the risk from your past exposure, because the virus cannot “reactivate” once proper PEP (ERIG + full ARV schedule) has been completed. After the wound is infiltrated with ERIG and you receive the vaccine series at the correct times, rabies virus is neutralised at the entry site and cannot progress later, regardless of future antibody fluctuations. Your antibody value comes mainly from the vaccines, not from ERIG. ERIG only works for 7–10 days locally at the wound and does not create measurable antibodies in blood. The report showing results in EU/ml is correct; that is the standard unit for this test. The test timing is also appropriate, since measuring titers after dose 3 (around day 14) is commonly done to confirm response. You have demonstrated a strong response, so there is no need for an additional booster. You are fully protected from the recent exposure, and there is absolutely very minimal rabies risk as of now.
Health Tips
If you want long-term reassurance, you can check titers again after 6–12 months, but this is optional, not mandatory. Good luck!

Hi, I can completely understand your worry, especially when it comes to dangerous conditions like rabies. I'll keep it short and sweet. Your rabies antibody level of 3.7 is very good. You only need 0.5 to be protected, and your level is much higher than that. This level comes from the vaccine you took and not from ERIG. Your antibody level will not drop suddenly in a few days — it stays strong for many months. Even if it slowly reduces later, it does NOT cause rabies to come back. Once you take full treatment (ERIG + all vaccine doses), the virus is completely stopped and cannot reactivate.
Next Steps
Even after this, you feel your concerns are not satisfied, consulting is the best option. take care.
