It is possible to conceive while in lactational amenorrhea  Many times ovulation takes place without any menstruation.

Some time between 18 and 28 April you had sexual intimacy which was fruitful. So you conceived. The ovulation may not be regular and predictable for many months after delivery.

Yes it's absolutely normal'.Some times HCG value is slowly rising but can be a technical error as well.

This means you are pregnant. This is the usual rise of hcg that we see in pregnancy. If you are wondering about the very low value on 18th..that is very early pregnancy. Since you might be breastfeeding your ovulation can be quite irregular and must have been delayed this cycle and you got pregnant with it.
