My LDL is 143 mg/DL and total Cholesterol is 208 mg/dl. Rest all fine in Lipid profile Recently I stopped eating junk , fried ( even not at home ) and sugary food. From that day 13 Aug ...will continue for the rest of the life. Current Medicine metformin, Vitamin E and saroglitazar and good diet ...what else I need ?

You are already doing most of the right things. With grade 2 fatty liver, PCOD and an LDL of 143 with total cholesterol 208, your numbers are borderline rather than dangerous, and at your weight the problem is mainly central fat and insulin resistance rather than obesity. Diet alone will not clear a fatty liver, so please add structured exercise: brisk walking or cycling 40 minutes at least five days a week plus resistance or strength work twice a week. That single change reduces liver fat and improves both lipids and PCOD. Keep protein high (dal, eggs, curd, paneer, fish), take whole grains instead of maida and rice, and completely avoid fruit juices, sweetened drinks, bakery items and alcohol. A few tests are worth adding if not already done: LFT with SGOT and SGPT, HbA1c and fasting insulin, TSH, vitamin D and B12, and a fibroscan or liver elastography to see if there is any fibrosis. Continue metformin and saroglitazar as your treating doctor has advised, they help both the liver and lipids; vitamin E is fine to continue for a limited period. Recheck lipid profile, LFT and HbA1c after 3 months, and if LDL stays above 130 with your PCOD risk then a low dose statin should be discussed. Do get seen sooner if you develop swelling of legs, yellow eyes, vomiting or severe right upper abdominal pain. If you want, share the reports with me on a consult here (fee Rs 500) and I will plan the next steps for you.

As you have already avoided junk food, start doing exercise for at least 30min. And also get a thyroid profile test.
