Yes, tranexamic acid is actually the better drug of the two for epistaxis - Dicynene (etamsylate) has weak evidence, while tranexamic works well for nosebleeds, including as local application on a soaked gauze pack. It should be avoided if there is a history of clots, DVT, stroke, kidney disease, or if she is on hormonal pills, so it is not a blind self-injection drug. But most nosebleeds do not need any injection at all - firm pinching of the soft part of the nose for 10 minutes with the head bent forward stops the majority.
Next Steps
Use pressure first; tranexamic only if a doctor clears it. If bleeding lasts beyond 20 minutes, is heavy from both nostrils, or she feels faint - go to the nearest emergency for nasal packing. Repeated nosebleeds need Hb, platelet count and clotting tests plus an ENT nasal check. Happy to review the case properly on a Practo consult.
