I can understand your pain dear.look dear sitz bath treatment is not only if you are taking oral pain killers.ointment and doing sitz bath..there a lots of thing you have to do..apart of that..then you can manage your pain or fissure??or otherwise you have to come to surgical side. Tell me one more thing are suffering from constipation??

U need  good  analgesics If unable to manage with oral, then may need injectable If no benefit from meditation then small surgery is the option
