I am Dr. Shamoon. Two things are going on with your friend and it helps to separate them. The fever itself - two days, coming and going, with marked weakness and body ache - is most likely an acute viral febrile illness, and in India at this time of year dengue, chikungunya, malaria and typhoid all need to be considered before calling it "just viral". The pattern you describe, fever that rises and falls with severe body ache and profound weakness out of proportion to the fever, is quite typical of dengue in its early days; that matters because dengue is most dangerous not on day 1-2 but around day 4-6, when the fever starts settling and the platelet count falls. So this is the window in which testing is genuinely useful, not something to wait out. The second problem is the night shift. Working nights and being unable to sleep in the daytime is shift-work sleep disorder, and chronic sleep debt lowers immunity, prolongs recovery and by itself produces daytime weakness and body ache that can be mistaken for the illness dragging on. That part will not improve with any tablet - it needs the sleep environment fixed. One important caution for your friend, since fever plus body ache tempts everyone towards painkillers: he should take only paracetamol. Ibuprofen, diclofenac, aspirin and combination "body ache" tablets must be avoided until dengue is ruled out, because they increase bleeding risk when platelets fall. Antibiotics have no role in a viral fever and should not be started without a diagnosis.

Get fever profile done which includes dengue profile typhoid, malaria,cbp and crp. Don't use painkillers till the further test results. Use tab.dolo650 sos until then.for sleep issues I'd recommend chamomile tea before bed until the further consultation and symptoms need to be re assessed thoroughly

Intermittent fever with body ache and weakness for 2 days could be viral fever, dengue, or malaria — especially with disrupted sleep and possible mosquito exposure. Night shift work also weakens immunity and can worsen recovery.

Paracetamol 650 mg for fever (max 4 times a day) Rest, plenty of fluids Multivitamin- like zincovit one tablet once a day x 10 days

Most likely viral pyrexia but can be other causes which include dengue malaria (common in monsoon season) along with typhoid
