It not must for everyone. If one suspect ajy defect while anomaly scan at 18-20 weeks, is there any history in parents or past child theh it is required. After 22 weeks you can take this fetal echo. During pregnancy you can not do anything except at some advanced fetal medicine centres.

Not a must.But its better to get it done because if there is any heart problem in baby delivery is planned in higher centre with paeds cardiology facility.Moreover number of babies with sporadic heart disease i.e without any family history or risk factors have increased since last 5 years.
