Dull pain in left scrotum

I am having this dull pain in my left testicle for some time. My surgeon examined me and told there is no such problem and I should wear a tight underwear for scrotal support. He also asked to do a ultrasound and it turned out normal. Then from WHERE DO I HAVE THIS PAIN? This pain kind of increases when I wear a tight underwear and I have to adjust it to make the pain go away.
