Yes - the bitter taste on the right side of the tongue and the right-sided throat irritation that worsens when your acidity increases are almost certainly due to laryngopharyngeal reflux, that is acid and bile vapour reaching the upper throat. It typically gives throat irritation, frequent throat clearing, a lump-in-throat feeling and a bitter or metallic taste, and it is often one-sided depending on your sleeping posture. The yellow, sometimes foamy stools are usually fast transit with bile-rich stool or fat malabsorption from diet - with normal
LFT, normal CBC, normal ESR and no jaundice, this is not a worrying pattern; reducing oily and fried food usually settles it. Mild gastric and gallbladder wall thickening on an old ultrasound, with no stones now and normal
liver tests, needs no active treatment - a repeat ultrasound once in a year is more than enough. My advice: start a proton pump inhibitor such as pantoprazole 40 mg once daily 30 minutes before breakfast for 4-6 weeks, and if the taste and throat symptoms are strong, add a night dose of a prokinetic or an alginate syrup after dinner. Along with this, eat small frequent meals, finish dinner 3 hours before lying down, raise the head end of the bed by 6 inches, cut down tea/coffee, spicy and oily food, citrus, aerated drinks, smoking and alcohol, and reduce weight if applicable. If symptoms persist after 6-8 weeks, get an upper GI endoscopy with a test for H. pylori, and an ENT scopy of the larynx.
