I have been suffering from lower back pain since one year after I lifted a weight with bad posture. I have been into fitness. The report is attached someone please tell me if this is serious need surgery or can be treated with exercise.

What other complaints do you have part from back pain. Is there any radiating pain in lower limbs. Is there any difficulty in walking or any tingling numbness in lower limbs..  As per the report, there is disc prolapse at one level but no significant cord compression.
Next Steps
If there are no other symptoms, you can start with physiotherapy and exercises for back strengthening,  start hot fomentation,  take analgesic tablet, calcium and vitamin d supplement.
