Be aware of the risks involved. Discuss with the hospital of your fears. Ask the dentist where the surgery will be performed. It totally depends on how many and how much teeth are decayed.. Best get done a X-ray. Meet a dentist who can try treatment conservatively with out general anesthesia thought opting for the later may require depending on how nicely child tolerate.

Very young or uncooperative patients are generally advised dental treatment under General anesthesia in a proper hospital setting under the supervision  of a trained anesthetist. The quality of permanent teeth depends on a lot of factors but provided you maintain proper oral hygiene there are good chances the permanent teeth will be alright. Stop bottle feeding esp at night.

I agree with the option on your doctor, but u can refer other child specialist to confirm. For a child of 2 years,its essential to save the milk teeth. Cooperation is a problem in such a young child. Hence treatment under general anesthesia.

There are 2 kinds of anaesthesia in dental treatment. Local anaesthesia is used in almost all cases and is safe for all ages (unless documentary history of allergy or reaction) General anaesthesia is often advised for very young children who need to undergo a lot of treatment and would not be able to sit in the chair for long periods. It is safe if it is done under supervision of an anesthesiologist and in a hospital set up. To expect the permanent teeth to be healthy when the milk teeth are in a poor state is difficult. There is always a risk of infection and abscesses from the milk teeth which can cause infection in the permanent teeth and also poor development of the same.
