Clozapine is not effective enough

My mother has had schizophrenia for years. She has been prescribed clozapine 200 mg for the last 6 months, but her symptoms have not gone. She has lost some weight recently and regularly experiences constipation. She also faces extrapyramidal symptoms (EPS) sometimes. What should I do?

You should titrate dose of Clozapine in coordination with your psychiatrist. Maybe the dose is not sufficient. Hence symptoms haven’t resolved. Or maybe an add on medication is required in addition to clozapine. There are also medications to prevent EPS . Constipation may be addressed by giving her some laxative and drinking lots of fluids and eating high fibre diet.

Clozapine is the last resort in treatment of schizophrenia. Though clozapine Has It's own side effect it does not cause EPS. Clozapine resistant schizophrenia Is a challenge to treat. You need to consult an astute psychiatrist and attempt the same. Good luck

Definitely continue with your treating psychiatrist - dose adjustments can be made (and I want to emphasize that calling schizophrenia clozapine-resistant depends on whether she's taken it for an adequate time, adequate dosage) and there are most definitely options even if she doesn't respond to clozapine
