Hello sir.. pain indicates the cavity reaches innermost (pulp)layer...you need to go for x ray (IOPA) to rule out the extension of the cavity and then pulpotomy ( removal of pulp layer) should be done...delaying the treatment will worsen the condition like swelling in the gums
Next Steps
kindly visit dentist for furthur clinical examination and management

Hello! I am a PEDODONTIST,  since the age of child is very young and as you said full upper jaw teeth have cavities , i can assume your nephew is suffering from EARLY CHILDHOOD CARIES (Ecc). This infection from tooth can go to nerves and effect permanent teeth below. It can also lead to severe space infections as blood supply in children is high. It will also impact his nutrition and diet patterns. But don't worry, we have complete cure for this and we can also prevent teeth from further decay. But child will have to be injected a couple of times and may feel pain. So You have two options: 1. Full treatment under conscious sedation: it is a "laughing gas " that relaxes the child.  pain will be less but still there. Also it may take 4 or 5 sittings. 2. Full mouth rehabilitation under General anesthesia: this is a day care procedure done in one hour. All teeth are treated in single sitting. Almost painless and less trauma.
Next Steps
consult a PEDODONTIST near you immediately!
Health Tips
read about the options and decide whats best for your child. stop bottle feed of he is doing it and start brushing with pea size amount fluoride toothpaste.

Hello kindly consult a child specialist (pedodontist) this problem is very much controllable and curable, but please follow the doctors instructions once u go see him, these are milk teeth and baby will keep them for a other 3-4 years. So they are important to save and cure.
Next Steps
Do not bottlefeed the baby to sleep with milk bottle, either swap it with water once he's asleep and clean his mouth with with tooth brushing before sleep. Or cleaning the mouth with a gauze wrapped around the finger.
Health Tips
Avoid foods with fluctuating temperatures.

It is in the best interest of the child to consult a paedodontist. It is important to treat all the carious teeth to allow the child to function optimally with the milk teeth and build good foundation for the permanent teeth.
