Fractured my ankle it's been 8 weeks almost since surgery physio started but my joint is complete stiff unable to put pressure on fractured leg and maintain balance, fibula bone was replace by titanium plate and other side of ankle also screws are put for support near tendon area ANY execercise to loosen the still muscles ? I try walking under my physiotherapist but I am unable to bear weight at all on fractured leg, it's healed yet due to so tight muscles and stiffness in ankle joint. How will the stiffness go ? In order to try walking as per my ortho suggestion ? What should I do to loosen it

you're experiencing some challenges with stiffness and mobility in your ankle after surgery. As a physiotherapist, I would recommend the following exercises to help loosen the stiff muscles and improve ankle mobility: 1. *Ankle Alphabet*: Write the alphabet with your toes, starting from A to Z. Repeat this exercise 2-3 times. 2. *Ankle Circles*: Move your ankle in both clockwise and counterclockwise directions. Repeat this exercise 5-10 times in each direction. 3. *Toe Spreads*: Spread your toes as far apart as possible and then bring them back together. Repeat this exercise 10-15 times. 4. *Heel Raises*: Stand on the edge of a stair or step with your heels hanging off the edge. Slowly raise up onto your tiptoes and then lower back down. Repeat this exercise 10-15 times. 5. *Calf Stretch*: Stand facing a wall with one hand on the wall for balance. Step one foot back about a foot and a half, keeping your heel on the ground. Slowly bend your front knee, keeping your back leg straight, until you feel a stretch in your calf. Hold for 15-30 seconds and repeat 2-3 times on each leg. In addition to these exercises, I would recommend the following: - *Progressive Weight-Bearing*: Gradually increase the amount of weight you put on your ankle, starting with small amounts and increasing over time. - *Balance Exercises*: Stand on one leg, with the other foot lifted off the ground. Hold for 10-30 seconds and repeat 2-3 times on each leg. - *Range of Motion Exercises*: Move your ankle through its full range of motion, including dorsiflexion (lifting your foot up towards your shin), plantarflexion (pointing your foot down), inversion (turning your foot inward), and eversion (turning your foot outward). Remember to listen to your body and stop if you experience any pain or discomfort. It's also important to visit physiotherapist's

After an ankle fracture and surgery (with plates and screws), your joint capsule, muscles, tendons, and fascia all become tight and stiff because: You were immobilized for weeks. The body forms scar tissue to protect the area. There is reduced blood flow and movement in the joint. This is why your muscles feel tight, and you cannot put weight or balance properly. It is very common after such surgeries.
Next Steps
Exercises to help loosen stiff ankle muscles and improve mobility. Please do these under supervision or after checking with your physio to avoid excess strain on healing structures.
Health Tips
Joint mobilizations, Regular, gentle, and repeated active and passive movements, Gradual weight-bearing activities and Stretching and soft tissue release

Hi! Thank you for sharing your concern. It’s completely understandable to feel frustrated at this stage of recovery, especially with stiffness and inability to bear weight. You’re not alone — this is a common phase after a complex ankle fracture with internal fixation like plates and screws.
Next Steps
~~ Warm-Up & Soft Tissue Mobilisation -Warm compress to the ankle for 10–15 mins before exercise therapy. -Gentle massage or myofascial release around calf and ankle (if no contraindications from ortho). ** Ankle Mobility Exercises (3 times daily at least)*** •Ankle pumps (point and flex your toes) •Alphabet exercise (use your big toe to “draw” letters in the air) •Ankle circles (clockwise and counterclockwise) •Heel slides on bed to gently stretch the Achilles. ~~ Stretching -Towel stretch for calf and plantar fascia
Health Tips
- Avoid forcing movements. Let pain be your guide. - Be consistent but gentle — stiffness improves slowly. - Communicate progress to both your physiotherapist and orthopaedic surgeon regularly.

Apply icing do exercises as suggested by the physio and take ultrasound therapy with infrared to remove the stiffness that'll make the joint mobility easy.

Hi! I hope you’re doing your ROM exercises like the alphabet movements, as well as strengthening exercises like towel scrunches, and important stretches like the calf stretch. To help loosen up the joint before exercise, try warm water soaks or a hot pack for about 10 minutes. Also, start walking short distances as tolerated to slowly regain mobility. Don’t forget to check with your physiotherapist — they can begin joint mobilization to help release the stiffness further.

To loosen the tight muscles you need to do proper graded stretches . Your physiotherapist can also work of soft tissue release techniques around the ankle joint and the calf region . The immobility must have caused stiffness in the small joints of the ankle complex and foot as well . Your therapist needs to use graded manual therapy to release these joint. Hope this helps

Medical Clearance: Orthopedic surgeon has to clear for weight-bearing and physiotherapy. If not yet cleared, should not attempt weight-bearing exercises. Pain and Swelling: Persistent pain or swelling may indicate incomplete healing or other complications. These should be reported to doctor or physiotherapist. Weight and Mobility: At 118 kg (BMI ~42), extra care is needed for safe progression due to increased joint load.
Next Steps
1. Gentle Range of Motion (ROM) Exercises (Non-Weight-Bearing) 2. Stretching Tight Muscles 3. Manual Therapy and Mobilization 4. Strengthening (As Tolerated) 5. Balance and Gait Training (When Ready)
Health Tips
If stiffness and pain persist despite these interventions, or if there is any concern about healing, consult the orthopedic surgeon and physiotherapist for further evaluation and possible adjustment of the rehabilitation plan.
