Bp unusually high these days

2025-10-25 06:43:27
I am on telma 40 mg and my BP has been well controlled, but I don't know why BP is constantly at 145/90 since the past 3 days. I don't have any stress, in fact, I just started on medicine for OCD - SSRI and multivitamins No matter how many times I checked, it is always 140/90 .. I am worried about this spike- what can I do to bring down to my normal baseline level, which is 120/80

BP around 145/90 is mildly high. Since you recently started SSRI, a small rise can happen temporarily. No need to panic.
Next Steps
Continue Telma 40 mg daily. Check BP twice a day for 5 days. Reduce salt, caffeine and stress. If BP stays >140/90 after a week review with your doctor for dosage adjustment or any add on meds.

Ocd is a stress condition  140/ 90 is higher  side of bp. So hardly made difference 140/ 95. Better to go with medicine under md physician.

As such 140/90 is higher side of normal, unless you are diabetic.  Get blood sugar done. Try to relax adequately,  do some physical exercise regularly. Take a fat free, low sugar diet.

Hey this is not an emergency yet, but it’s real and fixable — don’t ignore it. A persistent BP around 145/90 on telmisartan 40 mg means either (A) measurement/behaviour factors, (B) incomplete drug effect/adherence, or (C) need for treatment intensification. SSRI usually doesn’t cause sustained hypertension (SNRIs like venlafaxine do more), but any new drug can affect sleep/stress and indirectly push BP up. Do this now — no excuses Check BP technique & pattern Sit quietly 5 min before measurement, back supported, feet on floor, arm at heart level. Measure twice, 1 minute apart; record average. Do this morning (before meds) and evening for 7 days and send me the log. Home average daytime BP ≥135/85 = true hypertension. If you only checked once in clinic each day, those readings can be misleading (white-coat or device error). Immediate practical fixes (today) Confirm you took telmisartan 40 mg daily and at same time; don’t skip doses. Stop any OTC NSAIDs, decongestants (pseudoephedrine), excessive caffeine, or energy drinks — these raise BP. Cut salt, avoid alcohol for now, increase water, short brisk walk for 20–30 min if tolerated. Improve sleep tonight (poor sleep raises BP). Drug interaction / SSRI note Which SSRI were you started on? Sertraline/escitalopram/fluoxetine rarely cause sustained hypertension. Venlafaxine/atomoxetine (not SSRIs) are the ones most associated with higher BP. If you’re on venlafaxine, we must re-evaluate medication choice. Multivitamins do not raise BP. Investigations to order (simple, outpatient) — get these within 1 week: Serum creatinine, electrolytes (Na, K) — to check renal function on ARB. Fasting glucose and lipid profile. TSH. Urine spot for albumin (UACR). If BP stays high, consider ECG. When to change treatment If your 7-day home average remains ≥135/85 (or clinic average ≥140/90) despite correct technique and adherence → treatment intensification is indicated. Reasonable next steps: add a calcium-channel blocker (e.g., amlodipine 5 mg once daily) OR a low-dose thiazide-like diuretic (indapamide or chlorthalidone) depending on your labs and comorbidities. Don’t add without a brief consult — choice depends on age, electrolytes, occupation (e.g., erectile concerns), etc. Don’t stop telmisartan abruptly. Red flags — come in immediately if: Severe headache, chest pain, breathlessness, visual blurring, sudden weakness or slurred speech. Symptoms suggesting acute kidney injury (reduced urine output) after medication change. Practical checklists I want from you (send these in a consult): Exact name of SSRI + start date and dose. Home BP log (morning/evening × 7 days). Any other medicines (OTC, nasal sprays, painkillers) and alcohol use. Recent labs if available. If you want a quick, safe plan now I’ll: review your BP log and meds, tell you whether to add amlodipine 5 mg (most common safe add-on) or to try lifestyle + observation, and write the exact prescription and monitoring schedule. Book a focused consult — WhatsApp me at Nine Three Two Six Zero Two Zero Five Three Six and I’ll reply fast.

Hello, I am Dr. Romain Rajan, General Physician. Your case has been allotted to me. I understand your concern. You have been on Telma 40 mg with previously stable blood pressure, and now your readings are around 145 by 90 for the last three days after starting an SSRI for OCD. A mild rise like this is common during the initial phase of antidepressant therapy or due to small fluctuations in salt, sleep, or hydration. 1. Advices Continue taking Telma 40 mg regularly at the same time each day, preferably in the morning. Reduce salt intake and avoid pickles, papad, and ready-made soups. Keep yourself well hydrated and ensure 7 to 8 hours of sleep daily. Practice slow breathing or meditation for 10 minutes twice daily; it helps counter medicine-related mild tension spikes. 2. What to Monitor Check blood pressure twice a day at the same time, morning and evening, and maintain a log. Note any new symptom such as headache, dizziness, chest heaviness, or shortness of breath. Track pulse rate daily for a week. 3. Management The mild rise in BP can be because SSRI medicines (like sertraline, fluoxetine, paroxetine, etc.) can sometimes cause slight and temporary elevation in blood pressure during the first few weeks due to mild stimulation or anxiety relief phase. Telma 40 mg is a moderate dose and may need a small adjustment if the rise persists. You may follow this plan: Continue Telma 40 mg for one more week while observing readings. If BP remains above 140/90 consistently, you can discuss with your doctor about either increasing to Telma 40 mg twice daily, or switching to Telma H (Telmisartan 40 mg + Hydrochlorothiazide 12.5 mg) once daily after breakfast. Do not make both changes together without medical review. Avoid excess caffeine or energy drinks during this time. Usually, within 7–10 days, as your body adjusts to the SSRI, BP tends to normalize again. 4. When to Consult a Doctor Immediately If BP rises above 160/100 even once. If you develop severe headache, chest pain, dizziness, or blurred vision. If there is swelling in feet or sudden fatigue. Every patient’s situation is unique. For accurate adjustment of blood pressure medicine with your SSRI, it is important to discuss the exact drug name, dose, and timing. 👉 You can reach out to me directly for this case and also connect with me in future for any medical consultation. I am available on Practo or on WhatsApp at 85271646 seven seven for a one on one consultation, where you will receive • A personalized BP monitoring plan • Safe adjustment of Telma and SSRI combination if required • Quick follow ups to stabilize your readings Take care, wishing you good health. Best regards, Dr. Romain Rajan General Physician
