B12 Deficiency treatment

2026-07-06 06:51:25
Hello Doctors, I have attached my blood test report. My recent Vitamin B12 level is 124. Could you please advise the best treatment for my case? Should I take Vitamin B12 injections or oral tablets? If injections are recommended, how many injections will I need and over what duration? If tablets are sufficient, what dose should I take and for how long? When should I repeat the Vitamin B12 blood test after starting treatment? Thank you.

A Vitamin B12 level of 124 pg/mL is significantly deficient and requires proper treatment. At this level, injections are strongly preferred over oral tablets as the initial approach — this ensures reliable absorption regardless of whether there is any gut or gastric issue limiting B12 uptake. The standard loading protocol is Methylcobalamin 1500 mcg injections given on alternate days for 2 weeks (6 injections total), followed by one injection per month for 6 months. After that, depending on the cause, you may be able to switch to high-dose oral Methylcobalamin 1500 mcg daily for long-term maintenance. If injections are not feasible, high-dose oral Methylcobalamin 1500–2000 mcg daily is an acceptable alternative — but it takes longer to correct the levels.
Next Steps
Get a CBC (complete blood count) done now to check for anemia. Repeat the B12 blood test after 3 months of consistent treatment to assess response. Also check your Vitamin D, iron, and folic acid levels as these deficiencies often coexist. Avoid self-treating with low-dose B12 supplements (500 mcg or less) — they are insufficient at this level. Please consult me directly on Practo for a detailed evaluation.

A Vitamin B12 level of 124 pg/mL is suggestive of Vitamin B12 deficiency and requires treatment. If you have symptoms such as numbness, tingling, difficulty walking, or severe anemia, injectable Vitamin B12 is generally preferred.A commonly used regimen is Vitamin B12 (1000 mcg) intramuscularly once weekly for 4–6 weeks, followed by monthly maintenance .
Next Steps
Consult me on practo. Please share your CBC report, MCV, symptoms, and dietary history for a more specific recommendation. Repeat CBC and Vitamin B12 levels after 2–3 months of treatment to assess response.

Injectable is good if you prefer because its provide rapid recovery. One injection per week ,total 5 injection and then start oral supplements of b12 once weekly upto 3 months.and go for test after 3 months for adequate levels.
