Anal and groin area itching
Hello Doctor,
I have been experiencing itching for quite a long time in the groin area (inner thighs near the penis) and around the anal region. The itching becomes especially intense when I go to bed, whether during the day or at night.
I have already taken antifungal medication (AF-150) and used clotrimazole (Candid cream), and Luliconazole but there has been no improvement.
There are no clear red rashes or ring-like marks, but the skin has become irritated and slightly darker due to scratching.
Could you please guide me on what this condition might be and what treatment I should follow?

This is most likely NOT a fungal infection (since proper antifungal treatment has already failed).
• Most likely diagnosis:
• Lichen simplex chronicus (itch–scratch cycle)
• Pruritus ani (chronic anal itching)
• Possible irritant/contact dermatitis (sweat, friction, soaps)

• No ring-shaped rash → fungal less likely
• Long duration + no response to antifungals
• Skin darkening → due to chronic scratching
• Worse at night → classic itch cycle / pruritus ani
Next Steps
• Wear loose cotton underwear
• Change underwear twice daily if sweating
• Avoid tight clothes / gym wear for long hours
• No harsh soaps, dettol, or antiseptics
• Do NOT scratch → worsens pigmentation + itch

If it occurs after you go on bed whether it is day or night so bed bugs have to looked for. Any history of recent travelling? Similar complaints in family members?
