The skin prepuce seems tight need physical examination, why this come back.?
If very tight than he need circumcision?
You can apply apply coconut or Vaseline to retract it back.if it happens than in few days it becomes smooth, hygiene to be maintained, clean the skin n apply coconut oil n pull to normal place.
Consult a general surgeon for physical examination.
A clinical examination is required to assess if the prepuce is too tight.
You didn't mention the age of your nephew.
In such cases there is always a danger of paraphimosis. So a circumcision is worthwhile.
