25M with weakness low weight low appetit
I’m a 25-year-old male (45 kg, 163 cm) from Delhi and have been facing several issues for a long time: constant weakness, low energy, headaches, eye strain, very low appetite, early fullness, poor digestion, and difficulty gaining weight. I also have hair fall and varicocele discomfort which prevents heavy exercise. Mentally, I experience anxiety, overthinking, poor focus, confusion, and sometimes unclear or fast speech when stressed. These symptoms are affecting my work and daily life. I want to know what tests I should get, which specialist to consult first, and how I can improve appetite, gain weight safely, and manage anxiety and varicocele. Please guide me on the right diagnosis and treatment approach.

Tests you should get Basic health + nutritional panel CBC ESR/CRP LFT, KFT Fasting blood sugar, HbA1c Thyroid profile (TSH, Free T3, Free T4) Vitamin B12 Vitamin D Ferritin + Iron profile Serum calcium, magnesium Lipid profile Testosterone (Total + Free), LH, FSH Cortisol (morning) H. pylori breath/stool test (as you had gastritis) For digestion & low appetite Ultrasound abdomen Stool routine test For varicocele Scrotal Doppler ultrasound ✅ 2. Specialist to consult first Gastroenterologist – for early fullness, low appetite, digestion issues. Endocrinologist – for weakness, low weight, hormonal issues. Urologist – for varicocele. (You can start with a gastroenterologist; they will coordinate further tests.) ✅ 3. How to improve appetite & gain weight Eat 6 small meals, not 2–3 big ones. Add easy calories: banana, peanut butter, paneer, ghee, curd, dates, eggs. Take whey protein (better than mass gainer) after meals, not empty stomach. For appetite: Cyproheptadine 2 mg at night (doctor-advised) Probiotic daily PPI (if gastritis persists) ✅ 4. Anxiety, overthinking, low focus Daily 20–30 min walking (safe even with varicocele). Breathing: 4-7-8 technique. Reduce caffeine. If anxiety is strong: SSRIs (Escitalopram/Sertraline) – psychiatrist-prescribed Short-term clonazepam only if doctor says Do sleep hygiene strictly. ✅ 5. Managing varicocele Avoid heavy lifting/straining. Wear scrotal support underwear. Take NSAIDs only when pain is there. If pain persists or semen issues → urologist may suggest surgery (varicocelectomy).⭐ Overall likely causes Nutritional deficiencies (B12, D, iron, protein) Gut issues (post H. pylori gastritis, dyspepsia) Stress/anxiety Possible mild hormonal imbalance Chronic under-eating → low weight → low strength
