Hello, I understand your concern
Ballooning of the foreskin during urination is often due to a tight foreskin (phimosis), which is common in boys under 10 years and usually resolves naturally with age. The ballooning occurs because urine gets trapped under the tight foreskin before exiting.
You’ve started a local steroid ointment (likely betamethasone or mometasone) — this helps soften and loosen the foreskin. Improvement may take 4–6 weeks of consistent use.
Surgery is considered only if symptoms are persistent or complications develop. Most cases resolve with conservative treatment and time
Next Steps
Apply a thin layer of the ointment twice daily.
Gently retract the foreskin only as far as it goes without pain — do not force it.
After applying, keep the area clean and dry.
Teach gentle cleaning during bathing — no aggressive pulling back of the foreskin.
Avoid irritants like scented soaps or bubble baths.
Health Tips
Visit Pediatrician if:
If there is painful urination, urinary retention, recurrent infections, or redness/swelling.
If no improvement after 4–6 weeks of ointment use.

Hi,
Don’t worry.
It’s normal.
You can keep retracting the foreskin mildly during bath. Be careful you have to do it very gently otherwise it could hurt.
