What to do when water gets struck in the middle of ears while bathing?and its painful. I tried shaking and tilting my head, creating vaccum using palm, it's painful

You might have impacted wax which is causing you discomfort. Rarely, it could be a fungal infection as well.
Use Soliwax drops -2-3 drops every few hours. Wait for a few days, if no relief, meet with an ENT specialist to clear the wax/debris.
