Urine ACR increased over 3 months
My father is diabetic having type 2 diabetes and taking meds for the same apart from others like for BP, diabetic neuropathy, etc. His urine ACR was around 385 in Dec 2023 but recently when we checked it came as 1300 something. I am worried whether it is related to kidney problem. In December, we did his complete health tests including lipid, KFT, LFT, eGFR, etc and almost everything was normal. In May last year, his whole abdomen USG also showed kidneys as normal except some cystic sac which when shown to our diabetician, she said everything is normal. So what can be the reason for sudden spike in the ACR in 3 months in spite of normal sugar and BP?

How much is the hba1c now And were ther any recent events like fever,  new drug Intake. He will need close followup of proteinuria And what r the current meds he is taking

Hi Yes, one can have kidney disease even if creatinine and GFR are found to be normal, especially in your father who is a diabetic (duration to be mentioned). In early stages of diabetic kidney disease diabetic nephropathy) creatinine can be normal and GFR maybe normal or even high and he can have proteinuria. ACR is a measure of this proteinuria. There are amazing medications to reduce this proteinuria and slow down his kidney disease. And yes kidneys can get affected even if current sugars are well controlled.
