You have mentioned unsafe csom so Ian guessing it’s cholesteatoma. They have the tendency to recur. Even at good centres the disease May require 2 or 3 revisions. Don’t worry surgery can cure it. The longer you wait the more are the chances of hearing loss and facial nerve damage. Even though ear discharge can get temporarily relieved from medication, surgery is the permanent solution
