Unhealed perineal wound multiple resutur

I gave birth to my first child 17months ago via vacuum assisted vaginal delivery induced labor. After few days as stool was passing through vagina rectovaginal fistula surgery was done. No fistula occurrences seen. CT with contrast and MRI confirmed also I did a transvaginal scan. No issues found. Perineal tear 3x4cm is present near to vagina little away from anus. Its unhealed since then. RegenD was applied for months. Plastic surgeon refreshed edges and restitched it reopened in 14days. Before that natural drying and applying Enzomaz CM granules and IR light therapy was done skin was formed but it broke again hence went for refreshing wound. Later got restitched by urogynecologist after using placentrix and debridace and megaheal ointments it use to bleed from that area when ever motion is passed. This time stitch broke in 5days. Now again underwent surgery recently stitches broke after 2weeks. Underwent co2 laser twice. Its still the same. Please help Im scared 2 sit n pas motion
