Uncontrolled crying and laughing.
My sister is 21. She had some problems of depression from 14. It has become intense from July 2019. From march 2020 she is having some suicidal thoughts and crying episodes. From 3 July 2020 she is going through crying episodes with laughing at the same time. She can't control her emotions at that time. Other times she acts normal but gets scared of little things. She is also having eating and sleeping problems. She hallucinates herself trying to kill her in many ways and tried to commit suicide. And she also have massive headache all day. Sometimes she even begged to kill her. She was taking medicines for bipolar disorder but that didn't work. She is now taking medicines for increased prolactin levels. But that isn't working as well. Today she cried 4 times. The last episode lasted more than an hour. Please suggest want to do.

You have mentioned massive headache, uncontrolled crying and laughing , hallucinations and other symptoms. I think she needs a CT SCAN or MRI Scan of brain. It maybe a neurological problem. Please consult with a Neurologist immediately. If he brain scans are normal then only show her to a psychiatrist. It maybe a part of BPAD or psychosis. But first we need to rule out any medical cause.

So sad to hear that your sister is not able to come out of the suffering... Hearing to your history, it seems that your sister is suffering from BPAD... Being a chronic illness, BPAD requires regular follow up and prolonged medications...    But before we come to any decision, it's better to get her evaluated completely and if required changes in medication can be done. It's better to take her to psychiatrist immediately as she is actively suicidal.
