Unable to make my wife conceive

I have been married from past 14 months and trying for baby. My wife's period are regular with 30 days cycle. We often have unprotected sex at the time of ovulation. But till date, no success. Once, she missed her period by one month but next month, period occurred and in one month, her period lasted for about 15 days. I have had a piles laser surgery last year in June. Also had hernia and bladder stone surgery when I was 3 years old. I have healthy diet. We once visited the doctor and she prescribed hormonal medication for my wife as well that we completed. My wife's age is 26 and she is healthy.I am not sure whether this is due to sperm count, quality or mobility or problem is with implantation. Please advise.
