Total knee replacement
Sir,Due to unbearable pain in left knee of my spouse,51 yrs,Female,since five years as per the advice of the Ortho surgeon ,planned for TKR of Left leg.Now blood result shows Hgb 9.9 g/dl and hence blood of one or two units to be given at the time of surgery as per them My query is whether in a such difficult situation of adding blood surgery to be done?

Surgery is recommended for the suffering of the patient. She is only 51 but if she is in severe pain, disability with compromised functions then knee replacement is the last option with the conventional approach. my approach to knee is focussed more on the patellofemoral joint and the muscle health and address the same by using principles of ergonomics, load management, lifestyle modifications and pain receiving measures (patella taping, orthotics, analgesics).

Blood is a important parameter , decide not only by the surgeon but also the anaesthetist. Better not to cut down or try to avoid blood related events as this could endanger patient and procedure safety.
Next Steps
If you want to avoid additional procedures at this time of COVID, you can postpones the TKR till things become more clear. But the blood requirement still remain same , provided patients hemp globing does not reduce further.
