if you are not taking any tablets already only not take anything new but if you already continuing some kind of medicines for thyroid then you have to continue the same dosage

Difficult to say, just based on this report and the limited history you provided. If you are on a dose of thyroid medicine, you should continue. If not, there is no need for thyroid hormone. This test should be repeated after two weeks, as t4 is a bit high.
