The T3 and T4 exist in 2 forms mainly one is bound to protein and other is unbound when u say T3 that means unbound and total means bound plus unbound. Same is applied for T4
Now T3 is relased from T4 in some amount that T3 under stress or severe illnes gets converted to rT3 i.e. reverse T3 and this goes and binds to normal T3 receptors and reduces the effect of T3 i think this makes your doubt clear if not you can consult me futher with the case of yours
