Thought - Speech mismatch
Dear Sir, My father has been facing this problem since a very long time and it has worsened recently. His thoughts and actions / thoughts and speeches do not co ordinate with each other. He intends to do or say something but end up doing or saying something similar but not the exact same thing. He is also unable to remember names, phone numbers, etc. Is this something to worry about? Please help.

He needs to be evaluated for memory disturbance & speech problem. Take opinion from a neurologist.
