For a year or so ive been having on and off feeling of me needing to puke,it comes and goes and i dont end up puking. the feeling is gross. whats wrong with me?

The feel to puke or nausea may be due many non medical reasons,
But if persistent then it may be due to Gastritis , Altered liver enzymes or some hormonal disorders.
It is likely that you've developed gastiritis. I would advise you to get dewormed if you haven't in 6 months, you may require antacids. These following steps may help:
- Don't skip meals, Have a healthy and well balanced diet.
- Reduce/ Avoid spicy,fried, fatty, junk and oily food
- Avoid/ Limit drinking coffee, tea and carbonated drinks to once a day
- Exercise regularly (minimum 30 mins everyday)
- Increase daily intake of water to 3 liters
- Avoid/Stop smoking and drinking if you have the habit
- Reduce stress through methods such as yoga/meditation.
- Avoid painkillers as it will increase your gastric problems at this moment.
