Just by looking at the reports Dr cannot assess the situation of the patient and plan any treatment. In such cases it is best to meet the Ortho spine surgeon in person and get a proper clinical examination done to correlate the MRI report with the patients problems and then plan for the treatment. The MRI shows severe changes and hence the patient might need surgery.
