Small Flesh Tissue coming out of Anus
Hello Doctors, My fiance who is living in remote Sikkim has developed a painful condition where a small fleshy tissue is coming out of her Anus.  While she goes for her nature's call this tissue comes out even more in length. After nature's call, this tissue decreases its size.  This is very painful and gives her extreme burning sensation. Nowadays she can't sit properly, nor walk properly and she can't even climb stairs properly due to pain and burning sensation. She cries uncontrollably due to pain.  I've attached the picture of the flesh under normal conditions.  Request you to please help. The nearest multispeciality hospital from her place is around 300 Kms which takes almost 10 hours in the hilly terrain. If condition worsens then she will have to go but plz suggest and advice what we need to do for this to stop this permanently.   She will be in Pune in August when we are planning a surgery for this. Plz provide a solution / medicine  till August. Thank You.  Rana Pratap

Hi from the above description that you have provided it looks like your fiancee is suffering from a thrombosed external hemorrhoid which is a very painful condition and if left untreated it might result in in sepsis. I need to ask you a few more questions in order to determine if she requires immediate surgery for if she can be managed by medications alone. Also if I have to prescribe medications I need to know the other health conditions so that the medicines don't cause any untoward side-effects. Please consult me once so that I can get to know your disease better in order to provide the best treatment Regards Dr.Harsha.N MBBS,MS ( Consultant General Surgeon, Gastrointestinal Surgeon) Bangalore, I am an experienced surgeon in conditions of the Anus and rectum ..
Next Steps
thrombosed external hemorrhoids can either be managed by medications in the initial phases or it might require emergency surgery if it presents late please do not neglect this and consult  me at the earliest
