Slight itchy on penis head and swelling

This is unusual with my penis, I'm feeling slight itchy sensation on my penis head since a week. I can see small rashes on it.

Just put some boric acid in bowl of hot water and clean it properly, then dry it with clean cotton towel. Apply few drops of coconut oil. Always maintain hygiene. It will be fine else you can see me at clinic.
