look the phimosis if has been there for days its not easy to get it back to normal manually you have to consult a surgeon for it. Then the obesity and blackening of skin above your neck indicate acanthosis nigricans which incates life style disorder like insulin resistance which can lead to T2DM soon. about your scalp disease if its psoriatic dont run after any natural remedy consult a dermatologist for long term management and to prevent flair of recurrence.
Next Steps
everying has been mentioned from consultation part. for obesity and insulin resistance do exercise daily 40 mins of cardio atleast and avoid all high calorie diet like chocolate, baked items , pudding, ice cream, colddrinks .. for your addiction to masturbation addiction its ok to feel the urge and be sexually active . Can do its till 10-15 times a month. If feel too exhausted avoid it . dont try appying any unnatural thing while doing your pleasure activity that can lead you to an emergency .
