Scrotum dryness and itchy with pain

Asked for Male, 26 Years

I am having pain in right abdomen and urination is burning. I have dryness and itchy in scrotum region with black dirt surrounding it and pain in legs and feet burning. Testicles feels heavy and can't sleep in night bcoz of pain in right abdomen. Please suggest me what kind of problem is this and please suggest any medicine for black fungus and provide me solution to it?
