My scalp started itching around 1 yr back. After that I started having heavy dandruff. I had to scratch my scalp to remove the formed layer of dandruff. While scratching the skin either gets red or bleeds. The layer which I get after scratching is white/yellow in color. Now I have hairfall and oiling hair itches a lot.

You seem to be having scalp psoriasis. This chronic recurring skin condition usually is worse in winter or in times of stress. You will need special salicylic acid based lotions to apply over night. In addition you may also need tar based shampoo. N/b uvb photo therapy should be useful.
Please contact a dermatologist who can examine you and prescribe the appropriate skin care products
