Dear Doctor,
I'm 35 years old female. Had ectopic pregnancy. My beta hcg was low 600. So, expectant management for a week and went with left tube removal though tube didn't rupture. I opted for it as it was anxious to wait and watch.
Now. I'm 10 days post surgery. I didn't have any pain or issues at the incision sight. It is nicely healed. Had on and off discomfort at pelvic. No fever or bleeding
1. Yesterday noon i had dal nice with prawns. Evening i had periods like cramping at lower abdomen and lower back pain. Pain level was 5/10. Managed to sleep. Morning pain completely gone. Is it healing or gas pain or something to be concerned.
2. My LMP was oct 18. Had surgery on Dec 4. When can i expect my next periods.
3. I feel pelvic pain/pressure while passing gas, stools or urine. Is it normal?I asked my gynec a few days after surgery about scan and b-hcg. She said it is not needed. When can i go for next scan to know everything went well on my surgery.
Please guide me. I am helpless.

Your symptoms can be mostly due to heavy and spicy meal
Just wait and watch for your period
Try to take less oil/ less spicy/ comfort food
For next few days

Hi
Firstly the pain would have been due to gas due to heavy meal, generally cycle starts in a month's time, and you will not need a scan as no other symptoms like fever or any discharge from the wound site
Decidual cast which is endometrial thickening due to an ectopic pregnancy is present..once it is removed no hormone is there to support which can shed with cramp like pain. Though its already 10 days so rule out other causes based on history and examination
Next Steps
usually if surgeon is sure for complete surgery and histopathology shows chorionic tissue
Dear patient, I understand your concerns post salpingectomy.
1. *Pain after surgery*: The intermittent pelvic discomfort and cramping you experienced could be normal healing pain or gas pain, especially after eating a heavy meal like prawns. Since the incision site is healed and there’s no fever or bleeding, it’s likely part of the recovery process. Monitor it; if pain increases or persists, get examined.
2. *Next period*: After surgery, menstrual cycles usually resume within 4–6 weeks. Since your last menstrual period was Oct 18 and surgery was on Dec 4, you may expect your next period in about 4–6 weeks post‑op (around mid‑January).
3. *Pelvic pressure with gas/stool*: Feeling pressure while passing gas or stools is common after pelvic surgery due to temporary bowel or muscular adjustment. It’s generally normal but should improve with time.
4. *Next scan & follow‑up*: Routine post‑salpingectomy scans are not usually required if you’re asymptomatic. A follow‑up β‑hCG was advised initially to ensure ectopic resolution; now monitor symptoms. If you have persistent pain or irregular bleeding, schedule an ultrasound
