Hello Earlier I had a clear skin but for the past few months I got acne hence the scars. I've been applying some home remedies but they have an effect for just a day or two. What could be the reason ? Any specific medication ? Also, is stress a reason or the hormonal changes ?

Your age , stress , hormonal changes everything leads to acne. You need medications to keep it under control for few more years.
