Regarding fitness certificate
As i checked myself for 1 month prior there was a clavicle fracture.but now it seems to be alright so i want to create my medical certificate n fitness .as my previous recently closed his clinic so i m unable to create that please help so as i will my duty again

Will be made once you visit an Orthopaedician, Govt or pvt, depending on your company's demand.
