Hi, I am Dr. Atul Kumar Nimawat MS ENT. Hello, thank you for your detailed history. I understand your anxiety, and let me break this down for you clearly Findings from Image: Enlarged tonsils with exudative white patches — consistent with chronic or recurrent bacterial tonsillitis. No signs specifically indicative of immunodeficiency-related oral lesions (e.g., oral candidiasis, Kaposi’s sarcoma, hairy leukoplakia). Your image and symptoms do not show hallmark signs of HIV-related oral disease.
Next Steps
for your peace of mind: Recommended Investigations: 1. HIV Combo Test (4th Generation) – Detects both p24 antigen and antibodies, conclusive 28 days post-exposure. 2. CBC and ESR/CRP – To assess general inflammation and immune response. 3. Throat swab culture and sensitivity – To check for resistant bacterial strains. 4. HIV RNA PCR – Optional if extreme anxiety persists (detects viral RNA early).
Health Tips
Your case is more consistent with recurrent bacterial tonsillitis, possibly due to: • Incomplete antibiotic response • Resistant organisms • Local irritation (air conditioning, smoking, dehydration) • You may benefit from: • A complete 10-day course of targeted antibiotics based on culture • Gargles (chlorhexidine/saline) • ENT follow-up to assess for tonsillectomy if ≥3–5 episodes/year
