Rashes on my elbows

Respected sir/madam, I m developing some sort of rashes over my elbow for past few years i never consulted any doctor regarding this Rashes are thorny typ its very hard wen pressed some granule type come outs which is dry some tyms it bleeds to wen i try to puncture it. I don't have any itching problem just it looks awkward carrying these rashes over my elbow i m attaching pic plz have a look and suggest me any type of ointments.
