From your history it is not clear if you have painful micturition (which is likely due to urethritis, uti) or you have constant pain in the genital region (which may or may not be due to neuralgia).. first this needs to be cleared.. Then a thorough local and per speculum examination by a gynaecologist to rule out any local cause.. Once this much part is done , then further referrals might be needed like a neurologist for neuropathic pain, a urologist who might ask to get further imaging to assess whole urogenital system.. And finally you mentioned about anal fissure.. That is simple, a local examination by a general surgeon will catch the diagnosis and will treat accordingly..

Hello, I would recommend you to do CT KUB and urine culture first to rule out UTI If it comes positive it's definitely UTI. If negative consult neurologist for confirming your doubt of pudendal neuralgia.
Next Steps
please share your previous reports. UTI is becoming leading infection due to over usage of antibiotics without doing culture sensitivity. Avoid using antibiotics like a food on regular basis without doing culture. please do show your reports so that I can help you more I'm happy to help you 👍
