Prolonged jaundice 37 days old
My son is born normal delivery, he had jaundice and treated with phototherapy at day 6.  His jaundice was 8 .8 at 15 days. Now it's still at 10. He feeds well and gaining weight normally and no other health issues. His LFT, TSH, URINE TEST, ULTRASOUND are all normal. Blood group of both mother and son are B positive. Peripheral smear, reticulocyte, cbc etc are all normal. Direct bilirubin 2, indirect 8. What can be the reason? How worried should we be? He is exclusively breastfed.

Direct Bilirubin fraction in your baby’s blood sample is marginally elevated .The investigations already done have ruled out most of the conditions causing elevated direct bilirubin.Biliary obstruction is one possibility but is out of question since USG is normal.Other potential conditions that can give such a picture are alpha 1 anti trypsin deficiency and galactosemia. A metabolic and genetic work up might help, but the decision for the same must come after the results are thoroughly analysed by the treating doctor
