I brush my dog 4-5 times a week once in a day but he is developing plaque and tartar in his teeth, especially his back teeth.
I gave him Dentastix from Pedigree once but I noticed some bleeding on the the stick as he was chewing on it.
Any suggestions on how I can help him remove the plaque?
P.S : He doesn't have difficulty eating hard treats like carrots, yak cheese balls, etc.

As you have already dentastix and also following routine care for your dog teeth then you need to visit any near by vet for checkup and get it cleaned by veterinary doc.
